Permission to Participate and Sports Physical forms must be updated on a yearly basis and are due at least one week prior to the start of the each season the athlete participates throughout the year.

All forms are available in the High School Reception Office as well as listed below.

Forms should be turned into the High School Reception Office or Athletic Ministry Office in the HS building.

Student-athletes will be eligible to participate only when all forms and fees have been submitted.

Athletic Fees

$75.00		MS participant per sport each new season
$100.00		HS participant per sport each new season
$75.00		HS state qualifier participant in post-season play
$150.00		HS Football participant each fall
