Permission to Participate, Concussion and Physical forms must be updated on a yearly basis and are due at least one week prior to the start of the first season the athlete participates in for the year. All forms are available in the High School Reception Office and here on the website. Forms should be turned into the High School Reception Office or Athletic Office
