Attention Athletes!

 

Sports physicals will be provided at the CPCS Bothell Campus for students
in 6th-12th grade next year on Saturday, February 4th, 2012, 9:00am-1:00pm

 

The cost is only $35.00

Please make checks out to Dr. Gerald Yorioka

 

This physical is valued at about $350.00 and will cover all sports participated in for the remainder of this year

and every sport season next year

Parent volunteers with and without medical experience are needed. PIP credit will be given for all hours volunteered!

 

ALL PROCEEDS ARE AWARDED TO A CPC SENIOR IN THE FORM OF A COLLEGE SCHOLARSHIP

 

Students need to wear sweats and athletic shoes, there will be some running included as part of the physical.

Students will not have to disrobe for the exam.
